New Patient Forms Thank you for choosing Elite Dental of Natick. Our practice is fully digital and all your patient forms can be completed through our secure Modento application. You will get a text message or email when you schedule your appointment that will direct you to complete these forms. This portal is secure and will protect your personal health information as it is entered and securely input directly into our patient management system. You can also download the Modento application in the Google Play Store or Apple App Store which will allow you to complete your forms, have phone alerts for when you have appointments coming up, and update your communication preferences.If you prefer to complete your paperwork on paper – no problem, we can do that too! Please open the attached paperwork that can be completed ahead of time and emailed to us or brought on your first appointment. Rather do it in person? We can do that too and will have printed forms available for you on your first appointment. We will also confirm your general contact details that day as well.
