Digital Forms and InsuranceYou will get a text and/or email with forms and insurance information that can be completed ahead of your appointment. Save time and paper and help us kick off your appointment smoothly and efficiently. 3. What to BringBring your ID, insurance card, and a list of any medications to your dental appointment for a seamless check-in process. If you have recent x-rays or a referral, please send us a digital copy of these as well. If you have any copays, please bring a form of payment. Appointment Day!4. ArriveIf you completed your paperwork: great! We kindly ask you to come a few minutes before your appointment. If you need extra time to complete paperwork, please arrive 15-20 minutes ahead of your appointment. Minors under the age of 18 should be accompanied by a parent to provide consent for any treatment. 5. Check-InUpon arrival, check in at the front desk with your ID and insurance card to begin your dental appointment promptly. 6.
